U.S. Passport/Notary Public Services

Please note: As of 2017, U.S. Department of State recommends 6 months validity on passport when traveling.

Passport Book:

Adult (age 16 and over)


    • Long form birth certificate with both parent’s names on it (original or certified copy OR old U.S. passport (even if expired)

    • If name is different from name on birth certificate or old passport, a marriage license or other proof of name change is required

    • Filled out form (unsigned)

    • Photo (not stapled to form)

    • Check or Money Order for each passport

    • Driver’s license or other Government issued identification

    • $25.00 cash or check for a U.S. State Department mandated processing fee

    • Must renew by mail if the passport expired within 15 years of issue date and over 16 when obtained


Child (under age 16)


    • Long form birth certificate with both parent’s names on it (original or certified copy) OR old U.S. passport (even if expired)

    • Both parents must be present OR notarized consent form with copy of absent parent’s license, front and back

    • Filled out form (unsigned)

    • Photo (not stapled to form)

    • Check or Money Order for each passport

    • Driver’s license of each parent

    • $25.00 cash or check for a U. S. State Department mandated processing fee


Passport Card:

The U.S. Passport Card can be used to enter the United States from Canada, Mexico, the Caribbean and Bermuda at land border crossings or sea ports-of-entry.  The passport card cannot be used for international travel by air.  The passport card requires the same documentation as the Passport Book.

You are able to apply for a U.S. Passport at WWML during our regular Library Hours including some weeknights (no Saturdays).  We do ask that you please call us in advance of arriving at the library for an appointment.

  • WWML no longer takes Passport photos. 
  • Due to new government regulations, passport processing now takes between 8 - 10 weeks.  
  • Expidited service for Passport Book available for additional $60 fee.  Passport will be processed within 3-days of the State of CT receiving the application.  **Expidited service not currently available in the State of CT for Passport Card.

Get a Library Card

Do you live in Stony Creek? Area residents may obtain a free library card with proof of residence. If you live in another part of Branford, you may also get your Blackstone Library card here at Willoughby Wallce Memorial Library. A parent or guardian must sign a child's (under age 18) application. Cards are valid for 3 years. 

Borrowing Policy

Most books, audiobooks, DVDs, CDs and magazines are loaned for a period of 21 days. New bestsellers may be loaned for 7 days. Most items may be renewed one time, either in person, online or by calling us at (203) 488-8702 if no other patrons are waiting.
